How do I contact my provider?
If you are a family member (POA)
please call the facility that your loved one resides in and request
from the DON (Director of Nursing), the day in which your physician
or physician extender will be in the building again. Morning or
Afternoon? Do I need to make an appointment? At that point, you
should know what day to show up and then you will be able to meet
with your doctor or mid-level to discuss your concerns. The Business
Office is not able to accept patient or family member calls to
facilitate meetings with your physicians.
If you are a patient please
contact your nurse and request that she/he notify your physician
or physician extender for an appointment the next scheduled day
they will be in.
If you are a facility staff person:

If the call is urgent or life
threatening, please contact our Triage Department immediately

If the call is regarding business related
matters, please contact the Business Office at (336) 251-1114
and listen to the prompts for the correct party. |
If you are a funeral home:
 |
If you need the signature from
a Physician, please forward the death certificate to the facility
in which the patient had resided and the Physician will sign
it on their next scheduled day. |

If you need a signature for a "cremation"
death certificate, you may call the office at (336) 251-1114
and press 0 for the Operator, which will then give you the
location of the Physician where you may fax the death certificate.
