Once a patient enrolls or has been transferred to the care of Physicians Eldercare and before we can visit the patient, there are certain items that need to be addressed at time of admission, enrollment or transfer. Either by the patient or the financial and/or medical POA.
The patient/POA must sign all necessary documents allowing us to both treat the patient (referred to as a Consent for Treatment) and an authorization to bill the patient's respective insurance carriers, thereby allowing us to receive payment on the patient's behalf.
We will also require the following documents:

Copy of any power of attorney paperwork |

Name, address and phone of the authorized Medical POA |
